Updated guidelines issued Monday by the American College of Cardiology, American Heart Association and the Heart Rhythm Society call for Boehringer Ingelheim's Pradaxa (dabigatran) to be used "is an alternative" to warfarin to reduce the risk of blood clots and stroke in patients with paroxysmal or permanent atrial fibrillation.


The drug, which garnered FDA approval in December, can be substituted provided patients do not have a prosthetic heart valve, significant heart valve disease, severe renal failure or advanced liver disease, according to the guidelines.


Craig January, who headed the writing subcommittee for the guidelines, said that the opinion may help expand sales of Pradaxa, noting that "this is actually a very impressive roll-out for a new drug." However, he warned that physicians are new to the drug and some may opt to wait before prescribing the drug more broadly.


 According to Wa'el Hashad, Boehringer Ingelheim's vice president of cardiovascular and metabolic disorders marketing, more than half of the cardiologists that the company is targeting have written at least one prescription and while he declined to provide sales projections, he noted that "we expect this year to be a significant year of growth."


JPMorgan Chase & Co. analyst Chris Schott added that demand for Pradaxa suggests that there is likely to be room in the market for similar drugs under development, including Bristol-Myers Squibb and Pfizer's apixaban and Merck & Co. and Portola Pharmaceutical's betrixaban
